The Children with Disabilities team provides advice and support to families with children who have severe and complex disabilities. The team also aims to ensure that these children and young people are safe and protected from neglect and abuse.
Support may be in the form of a Care Package. Examples of this may include respite within a residential setting; or the payment of direct payments to fund a Personal Assistant to support your child to develop their independence.
We follow the assessment framework to complete a Single Assessment to identify the needs of the young person, and understand whether they will receive a service from our team. We will need to ask questions about your family, and ask for your permission to contact other professionals who work with your child. We may also be able to signpost you to other services which can meet the needs of your child.
If a young person is open to our service we will need to visit you and your child according to statutory guidelines, and carry out reviews to make sure the plans in place are working for your child.
Our team currently consists of 3 qualified Social Workers.
Tracey Annis is the advanced practitioner, who works 3 days a week Monday, Tuesday and Wednesdays.
Janet King who is part time and works the latter part of the week.
