At Age Care Advice, we strongly believe that each person’s journey during an illness or disability is unique and that people require consistent, trustworthy and individual support.

We offer the Opportunity of your own  Local Coordinator, Kimberly working alongside you during you or your loved ones’ experience and journey.

Kimberly is available to you 7 days a week for a range of services and 1 to 1 care.

Aspects of the service include, but are not limited to: 

  • Appropriate, monitored, responsive local Care Coordination
  • Helping promote your benefit uptake and continuing healthcare funding
  • Monitoring of your loved one if you take a break or a holiday
  • Guaranteed same day response
  • Support and advocacy when attending appointments such as memory clinic, GP, consultants, solicitors, dental, optical or hearing appointments
  • Capacity assessments
  • Finding the right care home or live-in care
  • Helping you find a tradesman such as a gardener or electrician
  • Attendance when you have someone come to quote or do work in your home
  • Help in realising the most support with a personal budget when working with social services
  • Representation if you are in hospital
  • Helping with a discharge plan when you are ready to leave to go home, along with monitoring you at home when you get there
  • Being the first point of contact on your carer’s emergency plan and/or with the lifeline alarm service
  • Risk assessments within 48 hours
  • Respond to lifeline activation

Kimberly is our Coordinator for Rutland, Melton and surrounding. Offering you personalised 1 to 1 care, helping with you with any but not limited to the below. Kimberly is available to you 7 days a week

Help, Advice and Support for:

• Helping you with your rights under the new Care Act.

• Navigating the care system.

• Finding the best care at home or a care home.

• Claiming your benefits and other funding sources available.

• Helping you develop a better understanding of the illness or

disability.

• Support with appointments such as GP or Memory Clinic.

• Physical representation if you are hospital. We will advocate for

you.

• Pulling in other professionals you may require i.e. Occupational

Therapist, Solicitor, Care Home Fees Planner or Psychiatric Nurse.

• Become an emergency contact for your Lifeline and Carers

Emergency Plan.

• Any practical problems that arise i.e. plumbing, electrical,

gardening etc

• Reconnecting you to your community.

• Find social outlet options that suit your personality.

• Can be purchased via your Personal Budget or privately

• Anything else that adds uniqueness to your situation, just ask.

• For those who self fund in care homes a monitoring/advocate

service.

• We never close your case.
