Supporting evidence and documents needed to apply for a blue badge

Types of evidence

You will need to send evidence to support your application. Some examples of evidence are:

  • letter confirming your diagnoses
  • patient summary or summary care records
  • letter confirming ongoing treatments, clinics or referrals
  • evidence of any medications you take for your disability or symptoms
  • Personal Independent Payment decision letters
  • Education, health and care plans
  • social housing letters or assessments from another team at the Council
  • care plans
  • evidence that your condition has progressed or advanced over time
  • evidence of any benefits your receive
  • letters from professionals involved in your care

For Blue Badge without further assessment applications

You will need to send us one of these documents, dated in the last 12 months, along with your application:

  • the letter confirming you're entitled to the higher rate mobility component of Disability Living Allowance
  • your Personal Independence Payment award letter with a score of eight or more points in the 'moving around' section
  • your Personal Independence Payment award letter with a score of 10 points for descriptor E in the 'planning and following journeys' section, on the grounds you're unable to undertake any journeys as it would cause overwhelming psychological distress
  • your blind registration card or certificate of visual impairment
  • confirmation that you receive the War Pension mobility supplement

You will also need to send us one item from the proof of address list, and one item from the proof of identity list.

Proof of address:

  • valid driving licence
  • benefit award letter (dated within the last 12 months) from the Department for Work and Pensions (DWP), Service Personnel and Veterans Agency, housing or pensions
  • Council Tax bill or utility bill, dated within the last 12 months

Proof of identity:

  • full or extract birth or adoption certificate
  • marriage or divorce certificate
  • civil partnership or dissolution certificate
  • valid driving licence
  • national identity card
  • valid passport

For Blue Badge without further assessment applications

You will need to send us one item from the proof of address list, and one item from the proof of identity list with your application.

Proof of address:

  • valid driving licence
  • benefit award letter (dated within the last 12 months) from the Department for Work and Pensions (DWP), Service Personnel and Veterans Agency, housing or pensions
  • Council Tax bill or utility bill, dated within the last 12 months

Proof of identity:

  • full or extract birth or adoption certificate
  • marriage or divorce certificate
  • civil partnership or dissolution certificate
  • valid driving licence
  • national identity card
  • valid passport

Photographs

Photos do not need to be taken by a professional or in a photo booth - you can take one with a digital camera or smartphone and print as a high quality image.

To meet the requirements, your photo should:

  • show you on your own
  • be taken within the last six months
  • be in colour, not black and white
  • be clear and in sharp focus, with a clear difference between your face and background
  • be taken against a plain, light background
  • be of you facing forwards and looking straight at the camera
  • not be torn, creased or marked
  • be printed as a high quality image
  • show you full head, without any head covering (unless you wear one for religious or medical reasons
  • be a close up of your head and shoulders
