Local health services

The RISE (Rutland Integrated Social Empowerment Service) team are part of Rutland County Council's neighbourhood approach to supporting people through social prescribing and other methods.

Social prescribers are health professionals based in GP practices. They work closely with GPs to assist patients with their non-medical needs by helping them identify 'what matters to them'. They work collaboratively to create a care plan, which may include referrals to local services and organisations within the community.
