Coming out of hospital

If you need minimal support

This is known as Pathway 0.

You may only need help with domestic tasks for a few weeks after planned treatment or a short hospital stay.

This could be provided by your partner, family or friends coming to stay, or a private agency.

Hospital staff will manage your discharge if you need only minimal help - for example with transport home or to switch on heating. They'll give you information about organisations that can help for a few weeks with tasks like shopping or cleaning.

Voluntary organisations like Age UK, Royal Voluntary Service and British Red Cross offer local services - find these and more on the Rutland Information Service website.

They may offer:

  • meals-on-wheels
  • lunch clubs and day centres
  • home visits and telephone befriending
  • handyperson schemes – help with minor jobs around the home
  • information and advice

In some cases, you may just need to restart the existing care you had before you went into hospital.

If appropriate, hospital staff will arrange for you to go home with your care provider or return to your care home.
