Assessing your care and support needs

Care and support plans

After your assessment, your assessor will work with you (or your representative) to develop a care and support plan.

This process helps decide the best way to meet your needs, and the plan will set out the details of what we've agreed.

During the planning process, we'll talk to you about personal budgets and direct payments.

Personal budgets

A personal budget is the amount of money we will pay towards any social care and support you need, if you're legally entitled.

Your personal budget allows you to decide how much control you want over arranging your own care and support.

Direct payments

A direct payment is an amount of money we pay you (or your representative) for the cost of arranging all or part of your support.

This allows you to take full control over your own care.

Care and support plan review

Once your care and support plan is complete and your support is in place, we'll set a review date to make sure your needs continue to be met.

If your needs change, we'll carry out a new assessment.

If you want to, you ask for your care and support plan to be reviewed before this date.
